Please read all information carefully. This form serves as your contract and by submitting it, you are agreeing to the terms of service listed on this page. At the bottom of this page is a list of measurements which you will need to submit, as well as fields for you to describe the clothing you're ordering. Please be sure to answer all applicable questions. All sections marked with an asterisk (*) are required. We will not be able to process your order without all of the necessary information.
PLEASE NOTE: As stated on our website, we can't always offer rush-order services. During our busy season, it can take up to EIGHT WEEKS to make our basic gowns and separates, starting from the time we receive this form back and can take even longer for more elaborate gown designs. Full payment must be received before we can begin work. If you have a deadline coming up before eight weeks from the date you return this form, contact us immediately so we can check our schedule to see if we can fit you in as a rush order. By filling out and returning this order form, you have agreed to the terms and policies listed below and on this page. PLEASE NOTE: It takes roughly four to eight weeks (starting from the date you return this form to us) to make our basic budget package gowns and separates, and longer for the more elaborate garments. If you are in a rush, please notify us immediately to make sure we can meet your deadline. Full payment must be received before we can begin work. By returning this order form, you have agreed to all of the terms and policies listed below and on our FAQ page. If you do not understand any portion of this form, email us and we'll be happy to help you. Please do not leave any sections blank, including the deadline for completion, name and address.
*never* send 'approximate' measurements - always have a friend help you take your measurements. Measurements must be precise and accurate, or your garment won't fit you.
*If you are dieting, please don't send 'hopeful' measurements
*Please don't make guesses as to how much of a cinch your bodice should have. Your bodice will not fit if you do this, because we always figure a cinch measurement into your bodice construction, and if you send us a measurement smaller than your *actual* measurement, your bodice will be too small.
PLEASE NOTE: Our dress forms adjust up to a 54" bustline and 45" at the waist. Apologetically, we are not able to fill custom orders for bust measurements larger than 54" and waist measurements larger than 45", unless the customer agrees to purchasing our mock-up fitting service to ensure an accurate fit. This service requires an additional 8 weeks, so be sure to order well in advance of your required delivery date. We cannot offer rush orders for those with a bust measurement larger than 54". Unlike other websites, we never charge more to construct a gown to a larger size, and any person, any size may purchase a mock-up service, but we do require anyone with a bust measurement larger than 54" and waist above 45" to submit a mock-up and purchase the mock-up service at $50. Please notify us before submitting this form if you need the mock-up service and we will be happy to assist you
If you choose to order a gown above these measurements and do not order a mock-up fitting, you will not be covered under any guarantees of fit, and all alterations will be your responsibility. Remember: All sales are final. There are no refunds.
When you write your delivery deadline below, please write this date on your calendar. This is the date by which you will receive your order (unless we otherwise contact you). We receive a lot of emails from customers asking the progress of their orders when there is nothing to report. We will contact you immediately if we're not able to meet your requested delivery deadline. If you don't hear from us, it means we were able to schedule your order to be completed by the date you requested, and that's the date by which you will receive your order.
Shipping Address: We are required to ship *only* to the confirmed address on your Paypal account if you paid via Paypal. If you have more than one address on your Paypal account, please look at your Paypal receipt to see which mailing address I will be using to make sure you have submitted the correct address.

Sometimes we can get silver grommets or black grommets, but we can't guarantee it. If silver or black are not in stock, we will use gold.

Please list all changes, details, notations, for example: "red dress, without antique lace pictured in this link:." Please do not write, "AS PICTURED" because often we don't have access to the fabrics used in the picture, and we will need to know the exact color you're requesting. Simple garments will not require very much information, while more elaborate garments will require highly detailed information.
Please do not write,"As discussed in emails." You need to write out the details and include them on this form since it serves as your final contract. Be sure to make notes of every detail, or it will not be added. This is the only form we work from during construction, so be very thorough and double-check your details. We do not work from past emails when doing construction, because very frequently, many changes are discussed throughout the email process. All *Final* decisions should all be noted on this form. Any details not mentioned on this form will be omitted from your order.

PLEASE NOTE: We do not offer hemming services for skirts. Hemming should always be done while the customer is wearing the gown, and to have the garment hemmed any other way may result in an uneven hem (hems are longer in some areas than others - like back side and hips, and are not always straight with the fabric grain.)
Because our clients sometimes ask that we keep as much fabric as possible on the skirt bases (for use for other projects, like hats, purses, etc), we can either serge-finish the base of your skirt, or leave the base unfinished. Serge-finishing cuts away some of the fabric and prevents the fabric from fraying, where unfinished will arrive with raw-cut fabric, but you will have more fabric to work with. If the base of the skirt is selvedge, we will leave it as-is.
